Manitoba Possible’s Children’s Service Coordinators work closely with families to identify strengths, needs, and goals, assisting families understand and access services and resources available to them in the following ways:

• Service navigation and facilitating referrals  

• Advocating within various systems like healthcare and education 

• Education on resources available to families

• Transition planning to daycare, school, and adulthood 

• Coordinating care through collaboration with other professionals like teachers and therapists.

Children and youth from birth to end of high school with a diagnosis of: permanent physical disability, permanent hearing loss, seizure disorder, or brain injury.
